Increasingly, modern periodontology and implantology face mistreatments, complications, failures, and even iatrogenic damages. It is therefore deemed valuable for colleagues and patients to share protocols, techniques, instruments, materials, and innovations developed to retreat these clinical cases with no room for error. All clinical cases described have only one parameter in common: a medical mistake.

Retreating these "special patients" requires passion, ethics, and the willpower to help patients who present with very high expectations and an exacting psychological makeup. Variables such as periodontal biotype, biologic and anatomic potential for tissue regeneration, and the nature of the underlying osseous architecture surrounding the ailing teeth or endosseous implants will influence the clinical decision making toward achieving optimal "biomimetic" results. The decision to perform implant treatment is ideally driven by the latest evidence-based information, in addition to the educational background, clinical experience, and access to technology of the periodontist; patient and site risk assessment; predictors of disease; economic aspects; probabilities and uncertainties of treatment outcome; and long-term stability of the results.

In der heutigen Parodontologie und Implantologie haben wir es immer häufiger mit Behandlungsfehlern, Komplikationen, Fehlschlägen oder sogar iatrogenen Schäden zu tun. Für Behandler wie Patienten ist es daher besonders wertvoll, etwas über Protokolle, Techniken, Instrumente, Materialien und Innovationen zu erfahren, die entwickelt wurden, um in einschlägigen Fällen eine nunmehr fehlerfreie Neubehandlung zu ermöglichen. Allen hier beschriebenen klinischen Fällen liegt ein therapeutischer Fehlschlag zugrunde. Eine Neubehandlung dieser "speziellen Patienten" erfordert Leidenschaft, Engagement und die Bereitschaft, Menschen mit einer sehr hohen Erwartungshaltung in einer psychologisch schwierigen Situation zu helfen. Variablen wie der parodontale Phänotyp, das biologische und anatomische Potenzial für die Geweberegeneration und die grundlegende knöcherne Anatomie im Umfeld der erkrankten Zähne oder der defizitären Implantate beeinflussen die klinischen Entscheidungen, die auf ein optimales "biomimetisches" Resultat abzielen. Die Entscheidung über eine Implantatbehandlung ist idealerweise evidenzbasiert, aber auch von der Ausbildung und klinischen Erfahrung des Behandlers und dem Zugang zu technischen Hilfsmitteln abhängig. Weitere Entscheidungsfaktoren sind patienten- und standortspezifische Risiken, Krankheitsprädiktoren, wirtschaftliche Aspekte, Erfolgswahrscheinlichkeiten und therapeutische Unsicherheitsfaktoren sowie die erwartete Langzeitprognose.

Volume 1: Endo failure - Tooth loss and perforation of the buccal plate
Band 1: Endodontischer Fehlschlag - Zahnverlust und Perforation der bukkalen Platte


Case description: A 45-year-old female patient presented with pain and no suppuration on the inferior border of the zygomatic right bone. Clinical examination and three-dimensional (3D) cone beam computed tomography (CBCT) revealed the presence of an endodontic lesion associated with a fenestration of the buccal bone. Simultaneous extraction, bone augmentation, implant insertion, and immediate loading were performed in one surgical phase to reduce the healing period. Only 4 months post-loading, the case was finalized after the pins that were visible through the mucosa were removed. Only two 1 mm incisions were performed to remove the pins without raising a new flap. The pins inserted next to the zygomatic bone were not removed, thus allowing for a minimally invasive surgical approach.

Fallbeschreibung: Bei einer 45-jährigen Patientin waren Schmerzen, jedoch ohne eitrige Infektion, am unteren Rand des rechten Jochbeinknochens aufgetreten. Die klinische Untersuchung und eine dreidimensionale DVT-Aufnahme zeigten eine endodontische Läsion in Verbindung mit einer Fenestration des bukkalen Knochens. Es wurden eine Extraktion, Knochenaugmentation und Implantatinsertion in Sofortbelastung in einer einzigen chirurgischen Sitzung durchgeführt, um die Einheilzeit zu reduzieren. Nur vier Monate nach Belastung wurde mit dem Entfernen der transmukosal sichtbaren Pins die Behandlung abgeschlossen. Dabei waren nur zwei Inzisionen von gerade einmal 1 mm erforderlich, um die Pins zu entfernen; eine erneute Lappenbildung war unnötig. Die neben dem Jochbeinknochen eingesetzten Pins wurden dabei belassen, sodass ein minimalinvasives chirurgisches Vorgehen möglich war.

Volume 2: Diagnostic failure - Mistreated trauma associated with periapical chronic lesions and anatomic bone deformities

Case description: A 57-year-old male patient presented with a discolored right maxillary central incisor due to a trauma he received about 20 years earlier. He reported that the tooth had been endodontically treated at the time. Since then, neither orthodontic nor prosthetic treatment had been performed. The patient's chief wish was to improve esthetics, although he had been told that not much could be done. The periapical radiograph, followed by a three-dimensional (3D) reconstruction, revealed the presence of an endodontic lesion, an atrophic basal bone, and an unusually large nasopalatine canal. Implant therapy was not an option; however, an advanced bone reconstruction was necessary prior to tooth extraction to avoid the complete collapse of the ridge in the esthetic area. A novel approach performing advanced tissue grafting to speed up the healing process is presented.

Fallbeschreibung: Bei einem 57-jährigen Patienten war der obere rechte mittlere Schneidezahn in Folge eines etwa 20 Jahre zuvor eingetretenen Traumas verfärbt. Der Zahn war seinerzeit endodontisch behandelt worden. Seitdem erfolgten weder eine kieferorthopädische Behandlung noch eine prothetische Versorgung. Der Patient formulierte sein Hauptanliegen so: "Ich möchte gern die Ästhetik verbessern, auch wenn mir gesagt wurde, dass die Möglichkeiten hierzu gering sind." Die periapikale Röntgenaufnahme und eine anschließende dreidimensionale Rekonstruktion zeigten eine endodontische Läsion, einen atrophischen Basalknochen sowie einen ungewöhnlich ausgedehnten Nasopalatinalkanal. Eine Implantattherapie kam nicht in Frage, doch wurde vor der erforderlichen Extraktion des Zahns eine komplexe Knochenaugmentation notwendig, um den vollständigen Einbruch des Kieferkamms in der ästhetischen Zone zu vermeiden. Es wird ein neuartiger Ansatz zur Beschleunigung des Heilungsprozesses, trotz weiterhin erforderlicher komplexer Augmentation vorgestellt.

Case description: A 78-year-old male patient presented with a crown fracture of a mandibular central incisor. The clinical examination and radiographic analysis showed the presence of multiple teeth with deep abrasions on the mesial and distal aspects. The patient reported that he had undergone nonsurgical periodontal therapy every 3 months as a supportive therapy, after having received periodontal surgery more than 20 years before. The periapical radiograph of the mandibular incisor showed a chronic periapical lesion due to the necrotic pulp, which was traumatically induced by recurring scaling and root planing. The endodontic lesion perforated the buccal plate so that an implant placement with a simultaneous guided bone regeneration (GBR) procedure was necessary.

Fallbeschreibung: Bei einem 78-jährigen Patienten war die Krone eines unteren mittleren Schneidezahns frakturiert. Die klinische und radiologische Untersuchung zeigte bei mehreren Zähnen tiefe Abrasionen der mesialen und distalen Anteile. Nach Angaben des Patienten wurde nach einer über 20 Jahre zuvor durchgeführten parodontalchirurgischen Behandlung alle drei Monate als unterstützende Therapie eine nichtchirurgische Parodontalbehandlung durchgeführt. Die periapikale Röntgenaufnahme des betroffenen Zahns zeigte eine chronische periapikale Läsion in Folge einer nekrotischen Pulpa, die traumatischen Ursprungs war, hervorgerufen durch wiederholtes Scaling und Wurzelglätten. Die endodontische Läsion perforierte die bukkale Platte, sodass eine Versorgung mit einem Implantat mit gesteuerter Knochenregeneration erforderlich wurde.

Case description: A 41-year-old female patient had undergone an implant treatment about 2 years previously. She presented with purulent exudate coming from the right nostril and from one of the dental implants placed after a maxillary sinus elevation. An endodontic lesion was also present next to the infected implant. The implant was mobile and painful. The treatment comprised only two surgical phases: the first phase aimed to eradicate the infection by removing the infected implant and endodontically treated tooth and closing the oroantral communication; the second phase was based on simultaneous implant removal, sinus elevation, simultaneous implant placement, guided bone regeneration (GBR), and immediate loading to reduce the healing time. This clinical case shows the potential for tissue regeneration even after iatrogenic implant dentistry.

Fallbeschreibung: Eine 41-jährige Patientin war zwei Jahre zuvor mit einem Implantat versorgt worden. Bei ihr trat ein eitriges Exsudat aus dem rechten Nasenloch und an einem der Implantate aus, die nach einer Sinusbodenelevation inseriert worden waren. Angrenzend an das eitrige Implantat war zudem eine endodontische Läsion zu verzeichnen. Das Implantat war mobil und schmerzte. Die Behandlung erfolgte in nur zwei chirurgischen Sitzungen. In der ersten Sitzung wurde die Infektion durch Entfernung des infizierten Implantats, Extraktion des wurzelbehandelten Zahnes und Schließung der oroantralen Fistel eliminiert. In der zweiten Sitzung wurde gleichzeitig das Implantat entfernt, ein Sinuslift durchgeführt und ein neues Implantat nach dem Prinzip der gesteuerten Knochenregeneration in Sofortbelastung gesetzt, um die Einheilzeit zu verkürzen. Dieser klinische Fall zeigt das Potenzial der Geweberegeneration auch bei iatrogenen implantologischen Problemen.
